- LPN License or CNA certification (competency test can be taken in lieu of CNA certification, score must be 80 or higher )

All items must be received by office before being scheduled for an interview.
Documents can be faxed, scanned/emailed, or brought in-person to be photo copied. Once you have turned in all documents to the office, call the office to be scheduled for an interview.
